The Guardian Council vets the candidates that run in elections. No one can run in the elections unless approved by this council.

How is this council formed?

The 12-member Guardian Council in Iran is chosen through a split appointment process controlled by the Supreme Leader.

The Supreme Leader directly appoints six Islamic clerics (theologians), while the remaining six civil lawyers are nominated by the Chief Justice—whom the Supreme Leader also appoints.

That means he has 6 in the bag and the person that elects the other 6 is also appointed by him.
He is not going to appoint people that are not going to do exactly as he says.
All 12 members are the supreme leader's handpicked fluffers of his.
These 12 members decide who can run in elections and naturally, they only choose candidates loyal to the eyeballs to the supreme monkey.
Even if they choose someone slightly not up to his liking, he can veto their choice.

The Council is responsible for vetting candidates for presidential, parliamentary, and Assembly of Experts elections, and ensuring legislation complies with Islamic law and the constitution.
This is what has enabled them to stay in power for 47 years. There is no fair and free choice allowed in Iran as long as the Ayatollahs have a grip on power.

That means the supreme leader can engage in all sorts of corrupt behaviour and misdirect the country's wealth and resources any way he wants while he has chosen 12 guys that back his every action or they are removed immediately. He can invent laws as he pleases and he can order killings on a whim (as he ordered the killing of 36000 people in January) and no one can stand up to him a dream of a chance to stay alive. He can order to kill and forgive the sins of the killers since he is pretty much god on earth....as he has been doing from day one.

That is how they started a brutal dictatorship and have held onto it since the beginning.
