Faced the same problem but turned out that the problem was from my bank as they had imposed an international spending limit on my card. Complain to FTMO live chat and if they can't do anything then hit your bank or card provider up.

Nope. Max loss doesn't reset monthly because your account balance doesn't reset either. It must never go below 10% of funded account balance (minus profits). On the other hand, daily loss limit resets every day.
