For some unknown reason I also see sometimes it isn't generated automatically. I think this is a rercent bug of mt4. You can manually compile it using mt4 compiler anyway.

See if ex4 is generated from mq4. If not, you can manually compile it. It looks mq4 is not being automatically compiled to generate ex4 when MT4 starts anymore.... Strange.
