MInimum account size shouldn't matter since FX scales. Unless you need to open with less than $400, MBT is the best options easily.

IBFX are horrible thieves, they stole from me like crazy. FXCM is a bucket shop but probably too big to really waste time manipulating individual accounts. MB is the one that won't screw with you at all.

There's a company called Tradesight that has a tool that does both the Sequential and the Combination. I have heard that they know a lot about Demark stuff. Might want to check with them.

You're talking deal desk language. On the MB Trading system, 1 = 10,000 units, so the $1.00 option you mentioned. So if you want 100,000 unit, you use 10. If you want 15,000, you use 1.5.
